With specific data, a group can design financial incentives based on fact, not generalizations. We are partners with you and your employees to describe what is going on with the group. We can analyze how many members have diabetes, heart disease, high blood pressure, high cholesterol and asthma. And we show your employees how to reduce the severity of these diseases over time.
We provide each member with gender and age appropriate wellness guidelines and follow up with individually tailored wellness plans.
We provide semi-annual wellness brochures to members.
All this can be linked to financial incentives. We can identify those members that have gotten their annual exam, or if they have diabetes, heart disease, high blood pressure, high cholesterol and/or asthma. We can tell if they are engaged with our nurse coaches. Based on this data, we can link financial incentives and discounts to members who are actively working on their health.
